  • Have you experienced a disruptive life event such as a break-up or divorce, the death of a loved one, or the loss of a job?
  • Do you feel angry, sad, frustrated, overwhelmed, or alone?
  • Do you just want to improve your mental well-being and live a healthier life?

Here's How To Get Started:

Step 1: Reach Out

The care journey begins by sign-up or referral. Once connected with Check-In, a trained "Care Coordinator" will be assigned to you or your loved one within 72 hours. The Care Coordinator will listen, answer questions, and begin the care process.

Step 2: Get Connected

After administering an initial intake screening, the Care Coordinator will develop a personalized care plan, matching you or your loved one’s unique needs to the programs and organizations best able to provide help.

Step 3: Receive Care

The Care Coordinator will then assist in scheduling care, with treatment options ranging from outpatient and telehealth programs to intensive, in-person care programs. The Care Coordinator will continue to serve as an advisor and guide throughout the care journey.
